Attendance Form

This Attendance Form must be submitted to the conductor prior to the missed rehearsal. In the event of illness or family emergency this form must be completed as soon as you are better or return from the emergency. For a tardy this form must be filled out by the day after the rehearsal. Failure to follow these rules will result in an unexcused absence.
